Upon discharge from the maternity hospital, you will receive the following documents:
- Maternity Exchange Card (second sheet): This document is intended for your women's clinic. It contains key information about the mother's health provided by the maternity hospital.
- Child's Exchange Card (third sheet): This is for your baby's pediatric clinic and includes important health data about the newborn.
- Third part of the Maternity Certificate.
- Part 3-1 of the Maternity Certificate: It is used to pay for medical services for the first six months of the child's medical monitoring.
- Part 3-2 of the Maternity Certificate: This part covers the cost of medical services for the second six months of monitoring.
- Birth Certificate Statement: This document is valid for one month after birth and is required to obtain the official birth certificate. It includes the date and time of birth, the child's gender, and the doctor's details.
- Birth Certificate.
Note: The birth registration application must be submitted within one month from the child's birth (paragraph b of Article 16 of the Federal Law of November 15, 1997, No. 143-FZ "On Civil Status Acts").Note for Mothers: Either parent can obtain the birth certificate at the local registry office (ZAGS) based on the place of registration of one parent or the place of birth. Along with the birth certificate, the registry office will issue a certificate (Form No. 24) required to apply for benefits. Recording the Child's Birth in the Parent's Passport: According to the Government of the Russian Federation decree of 12/20/2006, information about children who are Russian citizens and under 14 years old must be entered into the parents' passports by the Federal Migration Service. The record is validated with the official's signature and a seal. Registering the Newborn: A child under 14 years old can only be registered at the place of residence of the parents. To register your child, you will need the following documents:
- Both parents' passports;
- Marriage or paternity certificate (and copies);
- The child's birth certificate (and a copy);
- Application (Form No. 6) for registration at the place of residence;
- Consent from the other parent for the child to be registered at the spouse's residence.
Medical Insurance for the Newborn: This can be obtained at the place of permanent registration of one parent or through an insurance company that works with the children's clinic at your place of residence.To obtain the insurance, you need to provide:
- The child's birth certificate;
- The legal guardian's identity document;
- Pension insurance certificate (if applicable).
Note: The insurance policy must be issued within the first six months after the child's birth.
